The Diagnostic Value of Ultrasonographic Evaluation in Acute Appendicitis



Abstract
A prospective study was performed on 100 patients of suspected acute appendicitis during a 6 months period from October 1994 to March 1995. Clinical findings and ultrasonographic findings were compared. Clinical impression was classified into 3
groups:
highly suggestive appendicitis, suggestive appendicitis and appendicitis less likely. The ultrasonographic diagnosis was compared with each of the clinically diagnosed groups.
@ES The results were as follows:
@EN 1) Overall, ultrasonographic findings showed 81% of diagnostic accuracy, 72% of sensitivity and 87.8% of specificity. The negative laparotomy rate was 29.2%.
2) In the highly suggestive appendicitis group: Ultrasonographic findings showed 85% of diagnostic accuracy, 80% of sensitivity and 100% of specificity. The negative laparotomy rate was 10.5%.
3) In the suggestive appendicitis group: Ultrasonographic findings showed 75% of diagnostic accuracy, 68% of sensitivity and 82.6% of specificity. The negative laparotomy rate was 23.1%.
4) In the appendicitis less likely group: Ultrasonographic findings showed 87.5% of diagnosic accuracy, 66.8% of sensitivity and 89.7% of specificity. The negative laparotomy rate was 71.4%.
Ultrasonographic evaluation in acute appendicitis seems to be unnecessary in the highly suggestive appendicitis group. It has some usefulness in the suggestive appendicitis group and significant usefulness in the appendicitis less likely group.
